Susan Alger

Susan Alger, LCSW, CCM, is a mission-driven healthcare leader and rare disease advocate with over 20 years of experience in care integration across health plans, hospitals, and community systems. As Director of Care Management at Community Health Plan of Washington, she advances patient-centered strategies to improve outcomes and promote health equity.

Previously, as Senior Director at PacificSource Health Plans, she led enterprise-wide initiatives in care coordination, policy, and quality improvement. A rare disease patient herself, Susan brings lived experience to her advocacy, blending strategic insight with deep empathy. She has been actively involved with the Hereditary Angioedema Association (HAEA) for over 15 years, contributing to policy influence and efforts to establish Oregon’s Rare Disease Advisory Council.

Susan holds an MSW from Syracuse University and is a registered dance/movement therapist. An outdoor enthusiast, she lives in Eugene, OR, with her husband, two dogs, and co-owns a horse with her daughter.
