Why should you share your story?
Sharing stories is one of the most powerful ways that we, as a rare disease community, can change policy and save lives. Stories inspire, educate, and influence. Stories establish a greater connection to one another and help us remember and integrate what we learn. When a story captures the attention of policymakers, they are more likely to absorb the policy information we are conveying to them, more so than with facts and figures alone.

Simply complete the form below. If you are unsure of a question or it does not pertain to you, feel free to leave it blank. Everyone’s rare disease journey is unique. We invite you to include as much information as you are comfortable sharing.
What happens after you submit your story?
Thank you for sharing your story with us! Once submitted, it is added to our story bank, which our team reviews for themes and potential connections to current advocacy efforts. We may contact you if your story aligns with a policy ask or opportunity, such as outreach to Members of Congress. For your privacy, only your first name, last initial, city, and state will appear. Every story helps us better understand the rare disease community and strengthens our work together.
